Play Learn Bloom is registered with Medicare to provide services under the Chronic Disease Management Plan (previously known as the Enhanced Primary Care Plan). This funding is signed off by a GP for a child with a complex care needs. It involves a total of 5 allied health sessions in a calendar year.
